The other code violation that I came across involved the placement of photoelectric smoke detectors near HVAC vents.
As stated in the NFPA 72: National Fire Alarm and Signaling Code (2010)
I am referencing the NFPA 72 1999 edition since this is the version that most of our jurisdictions are still enforcing.
"2-3.5 Heating, Ventilating, and Air-Conditioning (HVAC).
2-3.5.1* In spaces
If you noticed the asterisk (*) next the to code section you would know to refer to the appendix in the back of the NFPA 72 1999 code book. To save you time it is on page #116 it it reads:
"A-2-3.5.1 Detectors should not be located in direct airflow nor closer than 3 feet (1 m) from an air supply diffuser or return air opening. Supply or return sources larger than those commonly found in residential ans small commercial establishments can require greater clearance to smoke detectors. Similarly, smoke detectors should be located farther away from high velocity air supplies."
In the picture below you will notice that the smoke detectors in this particular fire protection installation are too close to the air vents.
